What Is a GP Chronic Condition Management Plan and How Can It Help You Access Chiropractic Care?

If you have been managing a chronic musculoskeletal condition for six months or more — such as ongoing back pain, neck pain, sciatica, arthritis or chronic headaches — you may be able to access chiropractic care at Centrepoint Chiropractic Clinic with no out of pocket cost through a GP Chronic Condition Management Plan (GPCCMP).

What is a GPCCMP?

The GP Chronic Condition Management Plan — previously known as a CDM plan, EPC plan, Enhanced Primary Care plan, GP Management Plan or Team Care Arrangement — is a Medicare framework that allows your GP to coordinate your care for a chronic health condition. From 1 July 2025 all previous versions of this plan were replaced by the single GPCCMP.

If your GP determines that chiropractic care would benefit your chronic condition management, they can refer you for up to 5 Medicare-subsidised allied health visits per calendar year. These visits reset on 1 January each year.

Does Centrepoint Chiropractic Clinic accept GPCCMP referrals?

Yes. Dr David Chapman (Chiropractor) accepts GP Chronic Condition Management Plan referrals under MBS item 10964. We do not charge above what Medicare pays — meaning eligible patients pay nothing out of pocket for their chiropractic consultations under this plan.

What conditions may qualify?

There is no fixed list of eligible conditions. Your GP uses their clinical judgement. Common conditions that may qualify include chronic back pain, chronic neck pain, sciatica, osteoarthritis, chronic headaches and migraines. The condition must have been present — or be likely to be present — for six months or more.

How do you access it?

  1. Step 1 — Book an appointment with your GP and ask whether a GP Chronic Condition Management Plan is appropriate for your condition.
  2. Step 2 — Your GP prepares the plan and provides a referral to a chiropractor.
  3. Step 3 — Call Judy on 07 5495 1763 and let us know you have a GPCCMP referral.
  4. Step 4 — Bring your referral to your appointment. We process your Medicare claim through HICAPS — no gap fee.
