What is a GP Chronic Condition Management Plan?

A GP Chronic Condition Management Plan (GPCCMP) — previously known as a CDM plan, EPC plan, GP Management Plan or Team Care Arrangement — is a Medicare framework that allows your GP to coordinate your care for a chronic health condition. If your GP determines that allied health services including chiropractic would benefit your management, they can refer you for up to 5 subsidised visits per calendar year.

The GPCCMP is designed to support patients with ongoing musculoskeletal and other chronic health conditions, enabling you to access the support you need whilst keeping costs manageable.

Does Centrepoint Chiropractic Clinic Accept These Referrals?

Yes. Dr David Chapman (Chiropractor) accepts GP Chronic Condition Management Plan referrals at Centrepoint Chiropractic Clinic. The Medicare item number for chiropractic services under this plan is 10964.

We do not charge above what Medicare pays — meaning eligible patients pay nothing out of pocket for their chiropractic consultations under this plan.

Who May Be Eligible?

Patients with a chronic musculoskeletal condition that has been present — or is likely to be present — for six months or more are commonly eligible for GPCCMP referrals. There is no specific list of eligible conditions. Your GP uses their clinical judgement to determine whether a GPCCMP is appropriate for you.

How Many Visits Are Available?

Under a GPCCMP, you are eligible for up to 5 Medicare-subsidised allied health visits per calendar year. These 5 visits are shared across all allied health providers on your plan. For example, you might use 3 visits with a chiropractor and 2 visits with a physiotherapist.

Your 5 visits reset on 1 January each year, allowing you to access another 5 subsidised sessions in the following year if your GP determines ongoing treatment is clinically necessary.

Aboriginal and Torres Strait Islander patients may be eligible for additional visits under the plan.

What About the Recent Medicare Changes?

From 1 July 2025 the GP Management Plan and Team Care Arrangement were replaced with the single GP Chronic Condition Management Plan (GPCCMP) framework. If you had an existing plan before 1 July 2025 you can continue accessing services under that plan until 30 June 2027. New plans prepared from 1 July 2025 onwards use the GPCCMP framework. Your GP will be familiar with these changes and will advise you accordingly.

What was the GPCCMP previously called?+

The GP Chronic Condition Management Plan (GPCCMP) has been known by several names over the years — Enhanced Primary Care (EPC) plan, Chronic Disease Management (CDM) plan, GP Management Plan (GPMP) and Team Care Arrangement (TCA).
